Overview
Every character has a default look plus five skins earned the same way. One is a speedrun of the first Stage Boss, one a fast first crypt on the Graveyard, one a kill count, one a Tier 3 Final Boss kill, and the Gold skin comes at Rank 100. That covers 126 of the 133. The other seven are extras on a few characters, like Amog's Blue skin for 50,000 kills.
