Items are how you keep pace with the clock. Every second that ticks by scales monster health and damage up, so loot pulled off dead Lemurians and cracked chests has to scale with it, or a run falls apart somewhere past the twenty-minute mark. Most items are passives that kick in as soon as they land in your inventory; a smaller share proc off a kill or a hit instead. Picking the right ones, and enough of them, is the entire game.
Common (white), Uncommon (green), and Legendary (red) fill the normal drop pool, and each rarity up is a bigger payoff packed into a single copy. Boss items (yellow) only drop off teleporter bosses and stage-clear kills, so don’t plan around finding more than one or two in a run. Lunar (blue) and Void (purple) skip that pool entirely, and both charge you something extra for the benefit, whether that’s a permanent downside or a coin-flip curse.
Picking up more copies of an item stacks its effect, and most items scale in a straight line: Soldier’s Syringe adds another 15% attack speed every time, easy math. A few bend that curve instead. Rose Buckler’s armor is flat per stack, but armor’s own damage-reduction formula means each additional point matters less than the last one did. Ukulele runs the opposite direction, picking up more targets and more range with every copy you add.
